You will be part of the coordinator team together with a nurse from Home Care, assistance administrators and home care coordinator. The coordinator team will create a sustainable discharge process where each profession is used effectively to provide quality care to each individual with a focus on a safe and seamless transition from inpatient care to home. The coordination team must work together to ensure that the individual's needs for safety, continuity, coordination and security are met. The coordination team is responsible for all aspects of Lifecare from notification of admission to discharge. QUALIFICATIONS We are looking for you who are a licensed occupational therapist or physiotherapist. Experience of working with home care or similar activities is an advantage and if you have worked in Lifecare. Experience of administrative and logistical work may be an advantage. Great emphasis is placed on personal suitability as the work involves contact with patients, relatives and staff. As a person, you enjoy working both independently and in a team, have a good attitude, be flexible and have well-developed planning and collaboration skills.
