You will work independently and in a team with other medical professionals, in close collaboration with nursing and care staff, support functions and managers, as well as external collaboration partners. The work includes standard nursing tasks such as: - Carrying out medical assessments, planning care interventions and performing treatments - Providing care and support to patients in their homes - Proactive work - Documenting and following up patients in medical record systems - Interacting with doctors, rehab teams and other healthcare providers - Interacting internally with other social service providers - Provide support and guidance to nursing staff and students - Participate actively in development and quality work - Some stand-by duty may be required by agreement QUALIFICATIONS - Registered nurse/specialist nurse - Experience in home care or elderly care is an advantage - Requirement of B-driving license - Good knowledge of Swedish, both spoken and written - Ability to work independently and in a team - Empathetic ability and interest in the patient's needs We are in an exciting phase of developing our business.
